[nafex] annual meeting registration form

Hello everyone,

Several days ago I sent the registration form for the annual meeting.
I'm unable to get confirmation that it was in a usable format. So I'm
attaching it as a PDF.

Would someone please let me know this message and attachment came
through as for some reason I'm not able to receive my own messages
through ibiblio.
